D.A.T.E. FORM

• A foreman assigns a journeyman the responsibility to walk the crew’s work area each day – 4x’s per day, recording any safety concerns and / or housekeeping issues that will need prompt attention.
• Each and every hazard that is noted must be communicated to the foreman and corrected immediately.
• The D.A.T.E. form must be turned into the foreman at the end of the shift.
