D.A.T.E. FORM
- A foreman assigns a journeyman the responsibility to walk the crew’s work area each
day – 4x’s per day, recording any safety concerns and / or housekeeping issues
that will need prompt attention.
- Each and every hazard that is noted must be communicated to the foreman and corrected
immediately.
- The D.A.T.E. form must be turned into the foreman at the end of the shift.

- Hazards are immediately corrected at the field level
- Foremen are accountable for the proper delegation of work
- Employee ownership is realized as everyone shares the responsibility for safety

potential safety issue.

with all field personnel.
